Context

Our Care Coordination model has helped lead to dramatic life changes for many Memphians.

The vision for this program is to implement a collaborative, two-generational approach to address non-academic barriers of the whole family through a network of support, equitable access to community-based resources, and self-sufficiency skills to help families achieve their dreams. This work has been aided through our amazing partnership with United Way of Mid-South.

Care Coordination helps meet the needs of low-income families based on four core components: education, economic supports, health and well-being, and social capital. We’ve been able coordinate funds to assist with educational expenses, such as computers, tech equipment, or other supplies for parents enrolling in training programs or entering a job. The program has also enabled us to assist working parents with childcare and help families who received emergency eviction notices move into a new home. 

Youth experiencing homelessness receive more direct support thanks to CIS of Memphis’ Homeless Youth System Navigator, a new role added in December 2022. In addition to working within our Care Coordination Hub, this position collaborates with five other navigators across Memphis to ensure a more streamlined local approach based in collective impact. 

As we envision the future of Care Coordination, we’re working proactively to expand key funding partnerships to ensure more families are empowered to achieve a fulfilling future by gaining access to our services.

 

Approach

CIS of Memphis' Care Coordination Program empowers families to make seamless connections to critical community-based resources.

These resources or services are tailored to meet the specific needs of the whole family. Simultaneously, case managers ensure families are aligned with a network of support, have equitable access to community resources, and develop self-sufficiency skills to produce successful outcomes for children and expand opportunities for families to live a life of their dreams.
