Care Coordination
Integrated Student Support
Our Integrated Student Support specialists work to remove the non-academic barriers that prevent student success in the classroom.
- Partner with the school community to identify challenges that students face in class and at home
- Implement our unique model to coordinate with community partners to bring outside resources inside the school
- Help meet immediate needs ranging from food, clothing, and school supplies to health care, counseling, academic assistance, or a positive role model
College & Career Readiness
The College & Career Readiness (CCR) Program equips students to unlock their full potential through coaching, counseling, and supportive services that prepare them for any path after high school.
- Provides personalized guidance to graduate on time and ready to pursue college, vocational training/certification, the military, and/or career opportunities
- Exposes pathways for career exploration and skill development through the Work Experience (WEX) Program, including internships and part-time jobs while attending school
- Assists with training in soft skills, resume writing, job searches, and interviewing, as well as uniforms/work attire, work-related tools, expenses for educational testing and training, and more
Care Coordination
The Care Coordination model is a collaborative, multigenerational approach aimed to empower families to attain economic self-sufficiency.
- Helps meet the needs of eligible families based on four core components: education, economic support, health and well-being, and social capital
- Addresses non-academic barriers of the whole family through a network of support, equitable access to community-based resources, and self-sufficiency skills
- Provides assistance with educational and work-related expenses, such as computers and tech equipment, childcare for working parents, and housing for families affected by emergency evictions
